Seven out of twenty children's records were missing health statements. The Director is going to correct this by getting the health statments.
The caregviers were not having the parents sign their children in and out. The caregivers were doing this and putting mom, dad, grandma, or grandpa for children who were not even picked up by those people.

The substitutions for what the children ate for lunch and breakfast was not put on the menu. A photo was taken of the menu and their lunch. The Director corrected this at the time of the inspection by writing down the substitutions.
A child's napping on a nap mat was blocking an exit. A photo was taken of this. The Caregiver corrected this at the time of the inspection by moving the child to a different area of the room.
All play areas on the playground need the required amount of loose fill. Photos were taken of this. The Director is working on getting the loose fill and will have the caregivers up supervision until the loose fill is installed.
The operation had dead roaches on the floors after an extermination was done and the floors were not cleaned properly. There were also dead roaches found under a diaper changing mat due to not properly cleaning and sanitation.
It was found that caregivers were changing children's diapers on the outside porch leading to the playground. This is not a sanitary manner to change diapers.
There was one feeding instruction that had not been updated. It expired on 6/18/2021. The Director is going to correct this by getting the feeding instructions updated.
A caregiver left six 2 year olds on the playground alone for approximately 3 minutes while taking another child to the restroom. This was corrected by the caregiver going back out onto the playground.
At least four employees did not the proper four step sanitation process. The Director is going to correct this by having a training over the proper sanitation process.
One out of twenty four children's records evaluated was missing a health statement. The Director is going to correct this by getting the health statement.
In the three to four year old room there was a child that was napping in front of a door that was an exit to the outside. A caregiver correct this by moving the child away from the exit to another area of the room.
In the toddler room there were twleve 15-22 mo olds with one caregiver, they were napping. The SAG for the group was 18 months. The Minimum Standards state that you cannot reduce the number of caregivers by 50% during naptime. The Director corrected this by putting another caregvier in the room and splitting the group-the 15-17mo olds with one teacher and the 18-22mo olds with one teacher.
