A child was observed having their diaper changed and the child's hands were not washed prior to being laid down for nap. This was corrected when it was addressed with the caregiver and the caregiver washed chilren's hands after diapering.

Diaper changing supplies were observed to be on and underneath a bench in the main classroom. The diapering supplies were accessible to children in care.
Multiple children were observed in a chair with a tray and safety belt for over 15 minutes after they finished eating. They remained in the chair for approximately 1 hour and 45 minutes. This was corrected when the children were removed from the restrictive device.
Two infants under 18 months old were observed to be combined in a classroom with multiple children ranging in age with the oldest child being 11 years old. There were more than 12 children in care at the operation.

There were two uninstalled air conditioning window units in the playground area. The playground also contained a chandelier, rusted items such as a chair and lawn equipment, and there was a pile of wood with rusted nails sticking out. There was also a hammer sitting in the middle of play equipment. There was a broken slide with a puddle of accumluated water on it. Play equipment had a lot of dirt and spiders webs on them. There was also a large spider observed with a spiderweb in the playground area. There was a fallen tree limb inside of the fencing of the playground area. There was siding on the building that appears to be damaged and has nails sticking out. This siding was located in the area fenced for the playground.
Infant feeding instructions were not updated within 30 days.
