It was observed that a bottle was in a crib while a child was sleeping.

During an unannounced inspection it was found fire drills had not been conducted since the opening of the operation. This was fixed when a fire drill was conducted during the inspection.

During the investigation a caregiver was observed aggressively grabbing a child by the arm and popping another child on the hand twice with force on the surveillance cameras.

During the investigation it was found children were in the care of an unqualified caregiver without direct supervision of a qualified caregiver.

During the investigation it was found the operation does not have a main phone line for parents to contact.

During the course of a DFPS investigation it was found a caregiver responsible for a group of children in care was using their cell phone to take selfies.

During an investigation it was found the permit holder is not familiar with the minimum standards.

Based on preponderance of the evidence there is sufficient evidence that children in care were neglected.

During the inspection it was found bottles were being rinsed out in the diaper changing hand washing sink. This was corrected at inspection when a new procedure to send bottles home to be washed was implemented.

During the inspection a caregiver was observed stepping outside of the classroom leaving the group of 9 children ages 1 1/2 to 6 unsupervised.

A television was found to be playing a children's show in a classroom with a child that was under the age of two years old. This was corrected at inspection when the television was turned off and the children engaged in other activities.

During the inspection four children's files were reviewed, 1 child's file was not available for review, 3 children's files were found to not have health statements.

Two staff files reviewed were found to have completed CPR training online without the use of a manikin.

While reviewing documents there was no sanitation inspection available for viewing.

While reviewing the operational policy it was found there was no duration of indoor and outdoor play available.

While reviewing the first aid kit there were no tweezers available.

While reviewing 5 staff files there were no notarized affidavits available for viewing.

While doing a walkthrough there were holes in the ground in the outdoor play area.
