During a review of immunization records it was observed that three of eight records were lacking current immunizations or lacking a copy of immunization records. A list of children was left with director of missing immunization records.

This standard was investigated as part of an investigation and found to be in non-compliance. During interviews it was determined that a child was left in a classroom for approximately 30 minutes unsupervised and another child had wandered out of an open door into the hallway unsupervised.
