Based on the video, a caregiver yanked a child by one arm while walking the child back to his mat. Further, the caregiver picks up the child in an unacceptable manner and places the child on the mat a second time by one arm and one leg. Note: The operation immediately terminated the caregiver.

Based on the interviews conducted, the operation admitted to having an employee that used a loud tone with the children. This employee was retrained and the director modeled in the classroom how to talk to the children in a loving, soothing tone. When the director noticed the employee was not taking the retraining serious is when the teacher was terminated. The operation was proactive when the director would model in the classroom for the teacher on how to redirect children without using a loud tone.

Based on the interviews and inspection, a room was observed to have dried food on the children's chairs, children's fingerprints and grime on the front of the refrigerator, a dirty mat and dirt on a play equipment in the room accessible to children. During this investigation, it was noted that a caregiver had a CPR, First-aid card that expired in November of 2021. This was corrected when the caregiver received training on April 25, 2022.
A caregiver/operation did not use good judgment when they did not notify a parent until the end of the day (around 3:30 pm) that their child had fallen outside around 10:52am hitting his head on a bench resulting in a bump/bruise on the forehead.
There were a few bottles in the infant rooms that were not labeled correctly with the first name and last initial. This was CORRECTED when the caregiver labeled the bottles.

There was one child observed in the 3 year old room blocking the exit door during naptime. This was CORRECTED when the child was moved to a different area.

Based on the interviews conducted, a parent was not notified of the incident about a child at pick-up due to a caregiver not communicating with the closing caregiver about the incident.
Based on the video footage observed on the date of the incident, the caregiver lacked self control when they removed the child from the shelf in an aggressive manner and sat the child down on the floor.

A caregiver turned her head to answer another caregiver while standing at a diaper changing table with an infant on the table. This resulted in the 14 -month -old falling approximately 3 feet to the ground due to the caregiver not having her hand on the infant while the infant was on the table. The infant was immediately picked up by the caregiver. The infant did not sustain a concussion, swelling or bruising to the head, just a red mark on the forehead.
An infant was observed to be sleeping in a swing. Note: This was corrected at inspection when the caregiver moved the child to the crib upon request.
An infant was observed to be waking from nap and pulling up to stand in the stacked crib. The top of the infants head was able to hit the bottom of the above crib.
Staff did not use good judgement when failing to intervene in two separate incidents in which the safety of children was risked.
