A 4-year-old room was missing a cover for an electrical outlet accessible to children. This was corrected when covers were placed in the outlet.

Based upon interviews, staff did not do a name to face after transitioning children back into the classroom from a bathroom break. This resulted in a child being left unsupervised in the hallway near the classroom for approximately 1-2 minutes until the child was found by the director.

A 3 year old room had a purse out that was accessible to children and a 4 year old room had purses stored in a lower cabinet and the latch was not locked allowing the purses to be accessible to children. This was CORRECTED when the cabinet was locked.
