A teacher was observed feeding two infants without holding them. The two infants were observed on the floor mat being fed. This was corrected when the teacher held the infant to feed.

The operation admitted to not informing a parent of an incident that occurred at the center immediately that placed a child at risk until they were able to find out what happened from the caregivers and video.

During snack, the children were served sliced cucumbers, ranch dressing and water, which only meets one of the three required components that must be included per the USDA Nutritional Guidelines.

Based upon the evidence collected during the investigation, two children were playing on a piece of equipment in the classroom incorrectly with three caregivers in the room which resulted in one child having to receive medical care due to the equipment tipping over. It was determined through interviews/video that the caregivers responded adequately and professionally after an injury occurred to a child.
Based upon the evidence collected during this investigation, a piece of equipment in the classroom was not properly secured according to the manufacturing guidelines. This resulted in the equipment tipping over on a child causing an injury. The center was proactive by bolting the play equipment against the wall.

Based on the video, it was determined that positive methods of discipline were not used when a caregiver yanked a child several times by one arm as the child was upset after morning drop off and was trying to walk away as the caregiver was talking to her. The caregiver was seen tossing the child inside of a tent area that had pillows located in the room. Note: The operation immediately terminated the caregiver.
Several bottles in the infant room were not labeled with the child's first name and last initial. This was corrected when the provider labeled them.
Three infants were missing an updated feeding instruction.
Parts of the Plan of Action was not being followed. There were no observations in classrooms or outside for the month of November.
Two different incidents happened at the operation where the children had to seek medical care. The first incident happened on 8-5-2022 and the second incident on 9-12-2022. These two reports were not reported to licensing until 9-20-2022.
A child was injured while at the daycare and sought medical attention on September 1 and the daycare did not report the incident to Licensing until September 8 which is past the 48 hours required.
Based on the video, a caregiver was seen sitting on her bottom with her legs crossed on the playground while approximately (12) 3-year-olds were running around and some playing on the playscape. A 3-year-old was bumped by another child while one was going down and the other one was coming up the stairs of the playscape resulting in the 3-year-old falling through an opening and landing wrong on her arm fracturing it in several places. The caregiver did not see the incident.
Based on the interviews conducted, a caregiver arrived to work under the influence of a Controlled Substance. The caregiver was never in the room alone with the children and was terminated from the job immediately that morning by management.
Three out of ten children's records were missing the statement of special needs. This was corrected when the director was able to get the parent to fill it in.
A 12-month-old climbed up on the table during snack time and as he was trying to get back down, he fell hitting his head and bruising the lower part of his back. The two caregivers in the room at the time did not see what happened so the video was watched. The operation was pro-active by having the caregivers do some training before returning to work.
A caregiver walked away from a diaper changing table twice with a 14-month-old on it to tend to a child who got off his mat during nap time. The 14-month-old was not injured. The caregiver was terminated by the operation for her actions.
Several rooms did not have the cleaning bottles marked and they were empty and not ready for use. This was corrected when a staff prepared the bottles and labeled them for the rooms.
A staff that was hired was in a classroom without receiving the eligibility notifications from the background check unit. This was corrected when the assistant director had the person leave the center.
A maintenance active playground checklist was not available for review.
