A two year old child tripped while holding a caregiver's hand. The caregiver brought the child to their chest by raising the child by up with one arm. Same caregiver was carrying two children, and they set the second child down releasing the child a minimal distance before the child was all the way to the ground. *The above instances were not discipline related.*

Based on the video, a caregiver grabbed a child by one arm and the child grabs the caregiver?s arm with the other hand and the child can be seen in the video being lifted off the ground as the caregiver is walking out of the classroom.

The operation staff failed to ensure that all children were accounted for while transitioning the children out of the classroom to go into the gym at the end of the day, resulting in a child being left unattended in the classroom for approximately 15 minutes. The safety straps in the highchairs located in the infant II room were observed to have an accumulation of food on them. This was corrected when a teacher came in the room to clean them. The shelf in the infant II room had an accumulation of dust with a few dirty toys. This was corrected when the shelf and toys were cleaned. A drawer in the 3-year-old room had a broken latch with a few unsafe items such as adult scissors and staple remover. This was corrected when the items were removed and put out of the way of children.

Based on the interviews and video, a 19month old child that was being transferred to another room ran ahead of the caregiver during transition time, which resulted in the child walking in between parents out one door that leads into the foyer and out the front door of the building while walking in between two different parents until the child?s parent saw him outside in the front of the daycare next to a parent.

The infant II room was over ratio by one around 1:10pm. The room had 5 infants and the youngest was 9months old and the oldest was 12 months old with one teacher. This was corrected when the director removed an infant back to the infant I room.
Caregivers at the operation failed to ensure that an item was inaccessible to a child after taking it away several times, resulting in the child retrieving it and placing it in their mouth, resulting in a choking incident.
One out of five staff records that were reviewed for the HASA did not have a current pediatric CPR/First-aid card. The training expired on March 12, 2023. This was corrected when the director called to make an appointment.
There was documentation on the computer showing that a fire drill was conducted on August 9, 2022, but it did not show the start time of the drill or the length of the evacuation.
Two purses belonging to caregivers were observed to be accessible and within reach of children in 2 classrooms causing a potential safety hazard for children. This was CORRECTED when the purses were moved up higher out of the children's reach.

A caregiver did not use good judgment when they did not notify a parent until the end of the day (around 5:30 pm) that their child was cranky, sneezing, crying, and had red eyes all day long. Furthermore, a caregiver used their arm instead of a thermometer to check if a child had a fever. Upon pick up the child was warm to the touch and had a febrile seizure in the car on the way to the urgent care.
When reviewing a child's administer medication form the date, time, and amount given was missing when on the child's form.
One out of the ten children's records was missing the required immunization records.
One infant was observed sleeping in a restrictive device. The child was removed from the restrictive device and placed in a crib to sleep.
Two out of the eight tricycles used by children are missing protective handle covers. This was CORRECTED at inspection when the assistant director removed the tricycles from the outdoor play area.
When reviewing a child's administer medication form, the employee who administers the medication's full name was missing on the form.
The director's file were observed to be missing the required training hours for: SIDS, shaken baby, abusive head trauma, and early brain development training records.
Four caregivers who work with infants were missing the annual training records for: recognizing shaken baby, sudden infant death syndrome (SIDS), and understanding early childhood development training.
Two out of the ten children's records did not have the available information required for the Health Statement.
The school age room teacher gave the wrong count during the inspection. The teacher gave a count of 19 children instead of 22. The teacher said she forgot to add a child on the list when the child arrived in the room. This was Corrected when the children were recounted by another staff in the room.
Based on the evidence obtained, a caregiver was seen covering a child's head during naptime.
The play mats and a sink located in 1 classroom was observed to dirty. Note: This was corrected at inspection when the staff used the 3-step sanitation process to clean the mats and the sink.
