A 3-year-old child ran out of the classroom into the hallway while the teacher ran after the child leaving the classroom unattended. A staff close by heard and saw what was happening and made the decision to go inside the classroom with the children.

Three infants under the age of 12 months were observed to have a type of attachment hook on their pacifier such as a beaded string and a small stuff animal while still in the crib. This was corrected at inspection when the director detached each one from the pacifier.
