The fire inspection report was not available for review. Last fire inspection was completed 03/06/24.

During the walk through it was found that a child was left unattended in the classroom. Note: The children were out of room having lunch and the director and caregiver was also out of the room supervising the classroom eating lunch, the child was inside the classroom unattended.

It was found that a former staff member was allowed to shadow the director for 8 days without a background check.

Based on the information obtained during the DFPS investigation the parents were not immediately notified of a child injury that required medical attention. The child stated that they were injured when they fell on a field trip and the director was notified but the parents were not notified until pickup concerning the injury.

Based on the information obtained during the DFPS investigation the incident report was not signed by the operation.

One out of seven staff files reivewed had CPR training completed online (National).

The operation administered medication to two children in care without a parent's authorization. The operation no longer administers medications and all parents have been notified of the change in operational policies.

I did not observe documentation that the fire extingusihers in the operation's two vehicles had been serviced in the last year.

I observed an outlet that was not enclosed with a protective cover in a preschool classroom. NOTE: This was corrected during the inspection.

I did not observe the center's name and phone number on the outside or posted inside the gray van used for transportation.

In the front infant room there was a diaper in a trash can that did not have a lid. Note: A picture was taken. Note: The trash was emptied during the inspection.

In the school age suite the light in the bathroom on the left did not work.

I observed one caregiver supervising 42 school-age children. The required child/caregiver ratio for school-age children is 1 caregiver with 26 children.

During the walk-through with the director, I observed a group of preschool children sitting and eating snack. There was not a caregiver with the group. I observed a caregiver leave the classroom next to the snack area and rejoin the group.

During the walk-through inspection, a caregiver told me that she had 11 children, but when I counted, there were 12 children in the group.

Interviews were conducted and surveillance footage was reviewed. It was determined that caregivers did not properly supervise children while on the playground and as a result, a pre-school-aged child was injured while inappropriately using playground equipment.

In the 18-24 month classroom there was a electrical outlet, accessible to the children, that was not covered. Note: A picture was taken and the outlet was covered during the inspection.

There were 2 caregivers with 42 school age children.

The menu stated that the children would have apples and chex mex but the children were served cheese crackers and oranges. Note: The menu was updated during the inspection. Pic was taken.

In the school age classroom there were 3 yr olds present and there was a electrical outlet that was not covered. Note: The outlet was accessible to the children.

Two staff members, present during the inspection, did not have documentation available to show that they had completed CPR/First Aid training.

The pre school classroom was out of ratio as there was 1 caregiver with 22 children. The children in the room were 3-7 yrs old. The Specified age group was 4 yrs old.

The pre school classroom had 16 children with one caregiver. The ages were 3- 11 yrs old but the specified age group was 3 yrs old. Note: This was corrected when one child was moved to another classroom.

The caregiver in the toddler room removed their gloves after diaper changing but did not wash her hands. Note: When pointed out the caregiver washed her hands.
