During the monitoring inspection it was observed that the van used to transport 5 children, four of them between the ages of 4-6 years old, was not equipt with any booster seats or forward facing child safety seat.

During the monitoring inspection cleaning supplies and Raid were observed in the toddler bathroom, under the sink, without a lock and hand sanitizer and cleaner were observed in the pre k room on a low shelf, within reach of the children in care.
During the monitoring inspection the infant's pack n play and sheets were observed to be dirty, with dirt, spit up and food crumbs. The floors throughout were observed to have hair, dirt and crumbs. The toys in every classroom were observed to have dirt on them. The trash cans and stools in and around the operation were also observed to have dirt and other things stuck to them.

During the monitoring inspection the pack n plays were observed to have sheets on them that presented an entanglement hazard as they were not designed for the pack n plays they were being used on.
During the monitoring inspection it was observed that there was a shelf that was hung in the infant room, over pack n plays, that was coming out of the wall and a falling hazard. There were also two storage cabinets in the bathrooms that were a falling hazard and needed to be secured to the walls. On the playground the tarp was also coming up from under the loose fill, presenting a potential tripping hazard.
During the inspection it was observed that children were eating breakfast without washing their hands first.
During the monitoring inspection the fridge in the infant classroom was observed to have thawed breast milk with a date from June marked on it, as well as the fridge having hair, and dirt or old food stuck to it, needing to be cleaned out.
After reviewing child/caregiver ratios, it was determined there was more than an 18 month age difference between the youngest infant and the oldest child in a group. The youngest child was eight months old and the oldest child was 3-years-old. This was corrected at inspection when the oldest child was brought to a different classroom, bringing the facility into compliance.

After a review of children's records, three children enrolled in care did not have immunization records on file for review.
Ceiling tiles in the infant and pre-k/school-age classrooms were separating from the ceiling. Paint was peeling from the wall in the 2-3 year-old classroom and in the pre-k/school-age classroom. The baseboards were separating or missing from the wall in the 2-3 year-old classroom. A portion of a wall in the 2-3 year-old classroom needed to be cleaned as stains were visible.
The First Aid kit was found to be incomplete. There were no scissors nor tweezers.
A toy box on the playground was observed to have shear points on it.
A feeding utensil (sippy cup) was observed sitting on the handwashing sink in the infant classroom. The sippy cup was immediately removed when addressed and this deficiency was corrected at inspection.
A rug in the toddler classroom needed to be cleaned as stains were visible. Sinks in the toddler and infant classrooms needed to be cleaned as stains were visible. Both sinks were immediately cleaned when addressed, bringing the facility into compliance regarding the sinks.
