The safety documentation for drills and the inspection of equipment was not available for review during the inspection. The office was rearranged and all documents could not be found at the time of inspection.

One child was observed in a crib that was labeled for another child. *This was corrected at inspection when the crib was properly labeled.

One staff was the only caregiver present in the classroom with children without proof of First Aid and Pediatric CPR Training.

One staff was the only caregiver present in the classroom with children younger than 24 months of age without the required training in SIDS/Shaken Baby, abusive head trauma and Understanding early childhood brain development.

Two of the 3 staff files reviewed did not have all of the documentation as required in this rule. The files were missing the following: *Hire Dates/1st day on the job *Statement signed and dated that staff received a copy of the Operational policies and Personnel policies. *Statement signed and dated that the staff attended Orientation training.

There was excessive dust build-up and spider webs on walls, ceilings and ceiling fans.

One child was present wearing an amber teething necklace. *This was corrected at inspection when the necklace was removed from the child's neck to be returned to the parent.
