At the time of the inspection the playground structure had a crack in a side panel that was small, but had a rough edge and a second panel was secured with bolts that extended 8 threads out from the structure. The bolts were on the exterior portion of the panel and not easily accessed by children, but would be reachable by children's hands. On the playground area there was a loose pile of 2 garden hoses.

Based on information obtained through investigation it was found a caregiver had been present in a classroom without a completed background check and without the operation having been notified of the background check results.
At the time of the inspection the loose fill on the toddler playground measured less than the required 6 inches at the base of the slides of the structure.
Based on information obtained through investigations 3 children reported that a caregiver squeezed their hand or shoulder when guided to a time out in a way that hurt them.
Based on the information obtained a caregiver did not relate to children respectfully by raising her voice and talking in the child's face while re-directing behavior.
Based on information provided by DFPS a caregiver was observed and said to have raised their voice to children in care to get them to behave.
Based on information provided by DFPS 1 caregiver did not use self control when they pushed another caregiver to chase a child who was running away from them. A second caregiver did not use good judgement when they witnessed a caregiver using excessive force in discipline and did not intervene or notify the operation administration.
Based on information provided by DFPS it was found that the director did not supervise staff appropriately when 2 caregivers were found Reason To Believe for Neglectful Supervision and Physical Abuse.
Based on information provided by DFPS a caregiver stated that for discipline blankets are taken away from children at nap time because they are seen as a privilege.
Based on information provided by DFPS a caregiver was using their personal cell phone during an incident where a second caregiver was inappropriately disciplining another child. The second caregiver did not maintain adequate supervision of the class while on their cell phone.
Based on information provided by DFPS it as found that 2 caregivers were found reason to believe for Neglectful Supervision and Physical Abuse.
Based on information provided by DFPS and video reviewed a caregiver was observed yanking a child in the classroom and holding the child's wrists tightly, to the point of leaving a bruise.
