In a review of the DFPS investigation, it was found that a staff member had conducted an inappropriate restraint on 2 separate occasions during a non-emergency situation by dragging the child out from under a couch and by picking the child up to escort the child to their bedroom, as well as picking the child up again on a different occasion to escort the child outside when leaving a facility.
