When reviewing children files, not all immunizations were complete.

When reviewing staff files, training records were not available for licensing to view.

During walk through of the playground, in the toddler play structure a hole with sharp edges was observed. Operation will not be able to play on the structure until repairs are made and proved by licensing.

During walk through of the operation, several infants under 12 months were observed sleeping with blankets in the crib. Blankets were removed from infants cribs during inspection.

During walk through of the building, in the infant room, infants under the age of 12 months were sleeping in a restrictive device. Infants were removed during inspection and placed in cribs.

Operation had not had a fire drill with in the month they have been operating. Operation will be requried to complete a drill with in the next day.

During walk through of the outside play area, it was observed the after school play structure not having a secure closure of a hole. Shelf in classroom was not secure. Equipment can not be used until repairs are done and licensing has proved.

During snack time in the toddler room, toddlers were served granola bars with no barrier between the snack and the table.

Operation did not have a health inspection for Licensing to view at inspection.

Operation did not have a fire inspection for licensing to view at inspection.

Opertaion did not have gas inspection completed.
