A child was placed in May but their child's rights form wasn't signed until June.

One child and two caregivers stated the child will play outside for about 30 minutes without supervision.

Foster parent admitted to transporting the child in care without a car seat while under the influence.

While transporting and caring for children in care, foster parent consumed alcohol and misused prescription medication, placing the children in care at risk of harm, while operating a motor vehicle.

Operation failed to timely submit the information required to conduct an initial background check.

A caregiver did not apply prudent judgment in allowing a household member to teach a child in care by actually biting the child to show the child a lesson on how not to bite another child.

All medication including over the counter and prescribed must be locked to prevent children having access to these medications.

Observed weed grass killer, mosquito repellent spray, calcium and lime rust liquid in the bathroom during walk through.

Medication was not locked, allowing access to children placed in the foster home.

A caregiver talked negative in the presence of children in care and did not use prudent judgment when stating she was going to drop off the children at the police station if CPS did not pick up the children. The caregiver also failed to read the child's service plan which states that the child has an ADHD and ODD diagnosis. Furthermore, the caregiver may have possibly used the "F" word in front of the children.

The caregiver called her husband instead of calling 911 after a child almost drown in the water.

Two children in care, a 2 yo and a 5 yo were not wearing a life jacket while in the swimming pool and the hot tub area.

Caregiver failed to supervised two children in care resulting in one child almost drowning in the hot tub.

A caregiver did not report a communicable disease of a child in care timely to Licensing.

Foster parent was positive for COVID on 9-8-2021 and this was not reported until 10-6-2021.

It was found that foster parent's son was being the caregiver of the child in care while foster parent was in the hospital for 3 to 4 days. The son did not have a background check done in Texas and was not an approved caregiver.

Over the counter medication and prescribed nebulizer medication were stored on top of the kitchen counter and were not locked during inspection.

A bottle of Lysol spray was observed within reach of children.

Caregiver tested positive for a communicable disease and this information was not reported to Licensing.

A caregiver placed an infant in a swing without buckling the safety straps, leading to the infant falling out of the swing

The caregiver regularly swaddled an infant while the infant was sleeping in the crib

The caregiver regularly placed an infant on it's side to sleep

A child in care's bedroom had a camera in it for supervision purposes but was not approved for in the child's service plan
