During the walkthrough a diaper change was seen. The caregiver did not wash the children's hands after changing their diaper.

During the inspection multiple nap mats were observed to be torn exposing foam.

The operation had only test the Carbon Monoxide detectors once in the 2023 calder year.

During the inspection I observed the television on during naptime in the older children's room.

The operation had only tested their Smoke Detectors once in the 2023 calendar year.

During the inspection 2 caregivers were found to not have their current First Aid with Rescue Breathing.

The operation had only completed 1 documented fire drill for the 2023 calendar year.

During the inspection a trash can with a used diaper inside was observed in one of the children's restrooms. The trash can had no lid and was within reach of the children.

During the inspection only 10 hours of annual training were observed for the director.

During the inspection multiple caregivers were observed using their cell phones during nap time for personal use.

During the inspection 4 Personnel Records were reviewed. Out of the 4, 3 were missing their employment begin dates, 2 were missing the statement of Personnel Policies and Center policies as well as the statement of Orientation, 1 was missing their Education qualifications.

Your operation did not complete the employees who left survey for 2022 in the required timeframe.

Out of 6 records checked 3 records were missing the full address for a physician or emergency-care facility, and 1 record was missing the full address for their emergency contacts.

The operation had not completed a fire drill for the months of February, March, May or June.

The operation has not checked their Carbon Monoxide Detectors all year.

The operation has not checked their smoke detector at all this year.

The first aid kit was missing the guide to first aid, cotton balls, multi-sized adhesive bandages and tweezers.

During inspection the carbon monoxide detector has not been checked or documented since December 2020.

During inspection there was a chicken hutch and 5 chickens observed on the playground in the reach of the children when they go outside. This was discussed with the operation.

During inspection the training records were observed for the employees and found that the employees have not completed the 6 additional topics as required.

During inspection the nap mats were observed to be torn and the stuffing was showing. The mats could no longer be properly sanitized.

During inspection the training files were reviewed and the four staff have not completed child maltreatment training.

Duriing inspection the emergency drills were reviewed and have not been practiced since Deccember 2020.

During inspection the grounds checklist was observed and had not been completed since December 2020.

The inspector arrived for monitoring inspection and there was a sign on the front door stating that the operation was closed for the week of 8/2/21-8/6/21. The operation did not notify CCR of this planned closure of 5 days.
