Supervision was not met when a child was able to wander out of the restroom without the caregiver's knowledge. The caregiver was unaware of the ongoing activity of the child she was responsible for and lacked visual and auditory awareness.

The caregiver did not show competency during the transition from the exterior playground to the classroom resulting in a three year old child being left unattended and unsupervised alone on the exterior playground. A secondary caregiver did not show competency during the transition when she witnessed the child hiding under the equipment and did not advise the primary caregiver.

Supervision was not met when a child was able to exit the building without operation staff knowing. Operation staff did not have visual nor auditory awareness of the child?s whereabouts and was therefore unable to intervene.
