Caregiver supervising a group of children during naptime did not provide accurate number of children in the classroom. Note: corrected during inspection.

I observe fence n rear section of the playground area leaning over unsecured and gate broken off leaving an opening and thus not providing a proper barrier. I observe large tree trunk and branches presenting a tripping hazard.

The caregiver in charge of the 3 - 4 year olds classroom did not have accurate number of children in care.

The mulch around the climbing equipment located in both playground areas must be maintained at proper depth.

One infant observed in care had a bottle that was not labeled with the child's first, and/or last name. The infant bottle was labeled with tape with the child's first name, and initial of the last name.
