During the inspection it was observed that both caregivers in the toddler classroom were on their cell phones rather than directly supervising and interacting with the children in their care.

During the inspection it was observed that the toilets used by the children had visible signs of dirt and did not appear to be effectively clean behind the toilets.
During the inspection it was found that the first aid kits kept in the transportation vehicles all had expired antiseptic wipes in the first aid kit. This was corrected at inspection when new antiseptic wipes were added to each vehicle first aid kit.
During the inspection it was found that one out of the four staff who provide transportation was overdue on their annual transportation training by 7 months. Their last completed transportation training was on 03/10/2022. This was corrected at inspection when the caregiver took the training required.
During an investigation it was found that a child was left on the daycare van, sleeping and unattended.
This standard was found deficient as part of a DFPS Investigation.
During the DFPS investigation it was found that safety precautions were not taken when unloading children from the daycare van, and a name to face check was not completed.
The driver did not conduct a walkthrough of the vehicle this resulted in a child being left alone in the van.
During the follow up inspection it was found that the infant classroom was out of ratio by one child. There were 5 infants (3m,7m,11m,16m,18) to one caregiver. Specified age group is 0-11 months.
During the inspection an infant (3 month) was observed sleeping wearing a bib. A soft stuffed toy was also observed inside the crib. Note: Pictures were taken.
During the insepction it was found that one infant 2 month is missing parent signature for Novemeber on the infant feeding instructions.

During the inspection it was observed that the outside fence has broken from the bottom creating a gap where a child can squeeze through.
Mulch on the school age playground was low. Additionally, grass was growing through the mulch.
