On the two's playground, the use zone for the rotating bikes was not adequate.

Three out of eight caregivers were missing required annual trainings. Note: Corrected when the caregivers completed the missing trainings.

Grass and weeds were growing through the mulch on multiple playgrounds.

A child with known food allergies did not have a an allergy emergency plan for review.

Three out of ten children did not have current immunization records on file.

Two nap mats in the three-year-old classroom were torn which made them unable to be properly sanitized. Note- This was corrected during the inspection when the torn nap mats were thrown away.

An 11-month-old infant was asleep in a crib with a blanket. Note- This was corrected during the inspection when the blanket was removed from the infant's crib.

A school age caregiver and a preschool caregiver, in two separate classrooms, did not know an accurate count of how many children they were responsible for. Note- This was addressed during the inspection when the caregivers recounted the children in care.

The director was missing 18 hours out of the required 30 hours of annual training. The director was missing training in the required topics of Preventing the Spread of Communicable Diseases, and Building and Premises Safety.

Three infant bottles and three infant sippy cups were not properly labeled with the infant's first name and last initial. Note- This was corrected during the inspection when the infant bottles and sippy cups were properly labeled.

Nine out of nine caregiver's records evaluated were missing annual training in Preventing the Spread of Communicable Diseases. Five out of nine caregiver's records evaluated were missing annual training in Building and Premises Safety.
