Two out of six transportation vehicles had incomplete first aid kits. One first aid kit was missing gauze, a thermometer, medical tape, gloves, and scissors. The second first aid kit was missing tweezers and cotton. Note- This was corrected during the inspection when the missing items were added to the first aid kits.

Based on the information obtained, there is sufficient evidence to suggest that a child was served a food that was identified on the child's food allergy action plan.
