The turf on the playground for the infants is coming up and is a tripping hazard to the children.

A caregiver who did not have a cleared background check was present at the operation and had access to children in care.

The operation does not have a current annual fire inspection as the last was conducted on November 11, 2020.

The diaper changing mats in the infant, toddler, and one's classrooms were torn and/or discolored.

Mulch on the school age playground was very low as it was missing in multiple areas and grass was growing through.

In the one's classroom, multiple sippy cups and bottles were not labeled with the children's names. Note: Corrected when caregiver labeled each bottle and cup.

The was a 14 month old child in a classroom with a 46 month old child.

A child was blocking the entrance door to the classroom during naptime. This was corrected when the child was moved fromt he entrance door and placed in another area of the classroom.

There was one cargiver with 5 infants aged 0-11 months old in the infant classroom
