During an inspection conducted on 10/31/2023, the operation's annual fire inspection was expired. Upon follow up, the operation did not provide documentation showing the fire inspection was completed.

The operation's annual fire inspection expired on 10/11/2023.

Six out of ten employee personnel files evaluated were missing 24 hours of required training.

In the Infant A classroom, a three month old was swaddled while lying in a crib sleeping.

During a follow up inspection, the director had five out of thirty hours of annual training for the last annual training year.

Based on information obtained, there was sufficient evidence to support a child was left in the classroom without adequate supervision.

The director did not have annual training on file for review.

Three out of ten children's files evaluated were missing current immunization records.

The operation does not have a current sanitation inspection as it expired on 3/9/2022.

One out of three transportation vehicles did not have a safety alarm that was working properly.

Based on information received during investigation interviews there is sufficient evidence to suggest that the operation did not inform CCL of alleged abuse or neglect in the operation.

Four children did not have emergency contact phone numbers in the transportation binder. Note- This was corrected during the inspection when the emergency contact phone numbers were added to the transportation binder for each child.

Two out of ten children's records evaluated did not have up to date immunization records on file.

There were two gallons of milk that were past their expiration date. Note- This was corrected during the inspection when the gallons of milk were discarded.

The operation did not document the fire drill that was conducted in July 2021. Note- This was corrected during the inspection when the fire drill was documented.

There were multiple toddler cups in the handwashing sink in the diaper changing area. Note- This was corrected during the inspection when the cups were moved to the food prep sink.

There was an expired medication that belonged to a child who is not longer enrolled. Note- This was corrected during the inspection when the medication was discarded.

The operation has not had an annual sanitation inspection conducted since May 2019.

Based on the information obtained, there is sufficient evidence to suggest that the operation was not properly ventilated when strong noxious fumes entered a classroom where children and caregivers were present.

Based on the information obtained, there is sufficient evidence to suggest that the combined infant classroom exceeded the maximum group size.
