Based on preponderance of evidence a child had access to an e-cigarettes while in care.
The is sufficient amount of evidence to support allegation of caregiver not supervising children while outside on the playground. Also another caregiver fell asleep while supervising children ages 2 to 3 during nap time.

During the follow up inspection a caregiver was observed in the classroom caring for 10 children without a cleared background check.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
Based on the evidence gather it is determined that the caregiver in charged of the school age classroom was mopping while the children were in care.
The operation did not have any documentation of fire drills.
The sheet in the infant crib was loose and did not properly fit the crib. This was corrected when the sheet was replaced.
Based on the information obtained, there is sufficient information to show that supervision was not properly maintained due to a child being left alone outside.
