One child with a diagnosed food allergy does not have a food allergy emergency plan signed by a health care professional.

The director is missing trainings in the following topics: -Understanding and using safe sleep practices and preventing sudden infant death syndrome. -Recognizing and preventing shaken baby syndrome. -Preventing and controlling the spread of communicable diseases. -Administering medication. -Preventing and responding to emergencies due to food or an allergic reaction. -Understanding building and physical premises safety. -Handling, storing, and disposing of hazardous materials.

In the Toddler 1 classroom, two sippy cups were not labeled with the children's names. Note: Corrected when the caregiver labeled the cups.

In the infant classroom, I observed two bottles sitting on the hand washing sink. Note: This was corrected when the bottles were removed.

Two out of six children's files evaluated are missing current immunization records.

The toddler 1 classroom was out of ratio as there was one caregiver with 11 children, ages 14 months to 2 years old.

In the infant classroom, a three month old child was observed sleeping in a swing.

Two out of six children's files evaluated were missing current immunization records on file for review.

In the toddler 1 classroom, I observed children under the age of two years old watching "Gracie's Corner" and other educational programs.

The director and one employee are missing CPR training that adheres to the guidelines as the hands-on training and manikin portion were not completed.

The caregiver did not have access to the attendance of each child in care. This was corrected when the manager presented with attendance of all children in care.

A caregiver was allowed to have direct access to the children prior to receiving the results of the FBI background check. Note- This was corrected during the inspection when the caregiver's background check was ran and the caregiver was sent home until the results of the FBI background check are received.
