The sheets observed being used prevents the mattress to be flat and fit snugly on the sides of the crib. This was corrected when the caregivers removed the sheets to change them during inspection.

One cabinet inside of the children's classroom was unlocked allowing toxic chemicals, specifically, Febreeze, to be left accessible to children. This was corrected by the director moving the chemical to make it inaccessible to children.
The operation had five trash cans inside of the classrooms that contained garbage in them without a lid or front cover allowing it to be accessible to children. This was corrected at inspection when the trash cans were removed from the classrooms.
One caregiver did not know the number children they were responsible for in the classroom.
There was an expired Epi Pen on file for a child with a known allergy. The Epi Pen expiration date shows 1/2023.
There was one employee file out of the ten employees that were viewed that did not have a notarized affidavit for employment on file. Note: This was corrected at inspection when the employee got her affidavit notarized while at the operation, and the affidavit was placed in her file.
There were four child files out of the ten files viewed that did not have updated immunizations on file. However, three of those children's immunizations were corrected at inspection. There is one child out of those four that is missing an updated immunization record.

During an investigation there was sufficient evidence to support a child was neglected.
The 2-caregivers present were both observed cleaning the classroom instead of supervising the children. The classroom door was opened, resulting in a child leaving the operation near a busy road for approximately 30 minutes, without their knowledge.
3 cribs, used by napping infants, were not labeled with the infants name.
The toddler classroom did not wash hands after coming in from outside play.
A bottle of bleach water was located on the back of the children's sink. This was moved by the caregiver when it was brought to their attention.
Out of 5 infant feeding instructions reviewed 4 had not been updated in the past 30 days. 2 of the forms had not been updated since January and one infant did not have a form.
During an investigation it was found that the 2:00 pm diaper change was not done resulting in children being in the same diaper for almost 4 hours. There was a substitute in the classroom that day that is no longer employed.
During an investigation it was found that a caregiver did not use positive method's of discipline when redirecting a child's behavior.
In 2 of 2 classrooms the caregiver was unsure of specific information regarding the group they were responsible for. In both classrooms the caregivers did not know the ages of the children and in one classroom a substitute was unaware of how many children she was supervising.
Children in a napping classroom were mis-behaving after being awake with not alternate activity given.
One caregiver was supervising 22 children with a specified age group of 3 years. There was only one other caregiver in the building that was in ratio in a different classroom.
