The operation employed three caregivers who were under the age of 18 and was not enrolled nor completed a childcare career program.
A caregiver who was under the age of 18 and attending high school was left alone with sole responsibility of caring for children.
The director assigned a caregiver assignments that did not match their skills, abilities, and training.
The operation created a high school diploma in an attempt to satisfy Minimum Standards caregiver qualification requirements. The diploma did not accurately reflect the education that the caregiver had.
Five children and one adult reported inappropriate discipline was conducted at the operation. Three children reported a caregiver grabbed their arm forcibly. One child reported that a caregiver bent his wrist back. One parent reported that her child cries when coming to school and seems fearful. One caregiver reported that an autistic non-verbal school-aged child was confined to a dark bathroom because she was crying.
Three children reported that time-out is used when children do not sleep at naptime. Three children reported that children have to sit in a chair for the duration of naptime with no activity provided. One child reported that he hates naptime because he has to sit in a chair the whole time.
There is enough evidence to determine that one of the two caregivers who were on the field trip didn't have a camp shirt or lanyard etc. to easily identify the teacher as part of the field trip.
