Binder taken during transportation routes was missing the following items: -Emergency medical transport and treatment authorization forms -Parent s names and telephone numbers and emergency telephone numbers Note: This was corrected during inspection when provider made copies of the admission forms which include the required information.

Operation has lesson plans posted in the rooms but they are not being followed. Plans do not have the dates they are designated for and lack activities to help children meet their individual needs and developmental level.

Five children's files were reviewed. Three files are missing an emergency contact, three files are missing health statements, one is missing emergency medical authorization, one is missing verification parent received policies, one is missing parent signature and one is missing emergency food allergy plan.

One out three personnel files reviewed was not available at the time of inspection.

Monthly inspections of the active play space have not been documented.

Someone at the operation tested positive for a communicable disease illness deemed notifiable by the Texas Department of State Health Services and operation did not report this information to Licensing within 48 hours. Note: This was corrected at inspection when provider provided required information to complete a self-report.

Playground has overgrown weeds and grass. passageway from the center to playground was observed with trash including a metal plate with a nail and a used water bottle.

A child was observed napping on carpet. Two out the two cribs used for napping were not labeled with the children's name. Mats were observed at the operation, however, they were not labeled with the children's names nor assigned number.

Allergy Information for a child diagnosed with food allergies is not posted in the classroom nor kitchen.

Outdoor area has overgrown grass and weeds.

Outdoor activities are not been incorporated into the infant-age children daily schedule.

Emergency Practices have not beend documented.

Emergency Plan for a child with food allergies is missing.

Operation has not developed an Orientation Training Curriculum for employees.
