Binder taken during transportation routes was missing the following items: -Emergency medical transport and treatment authorization forms -Parent s names and telephone numbers and emergency telephone numbers Note: This was corrected during inspection when provider made copies of the admission forms which include the required information.
Operation has lesson plans posted in the rooms but they are not being followed. Plans do not have the dates they are designated for and lack activities to help children meet their individual needs and developmental level.
Five children's files were reviewed. Three files are missing an emergency contact, three files are missing health statements, one is missing emergency medical authorization, one is missing verification parent received policies, one is missing parent signature and one is missing emergency food allergy plan.
