2 children were observed with blankets over their heads during naptime (2 different classrooms 3-4 years and 3 year old classroom).

A child was observed napping on a cot that was blocking the exit.

The 3-5 year old classroom had 20 children (7-3year, 7-4year, and 6-5year old) with 1 teacher. The specified age group is 4 years. Making the teacher be 2 over ratio. The 2-3 year old class had 15 children (9-2year and 7-3year) with 1 teacher. The specified age group is 2 years. Making her 4 over ratio.

A staff in the 3-5 year old class was using phrases like "hush,""Cera,""stop doing that," and "get over here" during the visit. Her voice was sturn, firm, and louder than her normal voice.

A staff in the 0-18M class did not follow manufacture's instructions when she placed a child (4M) to sleep on a bobby pillow.

Based on the information gathered during the investigation, it has been determined a parent was not given notification of an less serious injury to a child.

A child in care was neglected by operation.

Based on the information gathered during the investigation, it has been determined that child was neglected, and caregiver was not at a physical proximity that allowed visual and/or auditory awareness when the child was unsupervised.

The operation did not wait 24 hours to allow a child to return that had fever and symptoms of illness.

The operation waited 30 minutes to call the parents to inform them of the situation. The call was not made until the PD told the operation to call.
