A caregiver grabbed and pulled a chair by the arm. This resulted in a child receiving a broken arm.

This standard was evaluated as part of an Abuse and Neglect investigation. A child was grabbed and pulled by a caregiver. The child received a broken arm due to this incident.

There were missing safety straps from the bucket seats.

Two children's records did not have a health statement signed by a physician.

The operation did not evacuate the entire center when they noticed an electrical fire. They evacuated the area that was affected. The fire was put out immediately with a fire extinguisher by a staff member. 911 was called and the fire was put out by the time the department arrived.

The transportation folders did not have the emergency authorization information.

One of three vehicles used for transporting did not have a fire extinguisher. The vehicle did have a fire spray but an extinuisher is required.

Two diaper changing pads need replacing.

The operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility. The owner was informed to not allow the staff member to return until they had received the eligibility letter.
