The operation did not immediately notify a parent that their child had been left on the operation vehicle.

It was determined that the operation did not have physical proximity to and auditory or visual awareness of a child for an extended period of time.

A driver at the operation did not ensure each child was accounted for before exiting the vehicle.

It was determined that the child safety alarm on one of the buses at the operation is not in working order.

The last fire inspection the operation had documentation for was completed on 3/18/22.

The operation had not completed a fire drill since November of 2022.

The last sanitation inspection completed for the operation was 12/06/21.

The acting director does not have an eligible background check determination from the background check unit.

The acting director of the operation has been in the position since 10/24/2022 without any notification to licensing until 11/16/2022.

The operation did not have a recent fire inspection on fire for review only documenation of an invoice that was date 3/25/2022.

Eight staff who have been employed over 12 months did not have all of their required training hours. Training hours provided by the operation fro review were dated year 2020.

According to doumentation provided by the operation the operation had not conducted a fire drill since 8/2021. The operation was able to bring this standard into compliance by conducting a fire drill today.

The infant room was observed to have 13 infants present with 3 caregivers. The specified age group of the classroom was 11 months according to birthday documents of the children present.The operation did bring the standard into compliance by splitting the group.

Based on the information gathered during the investigation, it has been determined that child was able to walk outside the operation due to the caregiver not having physical proximity that allowed visual and/or auditory awareness of the child in care.

Based on the information gathered during the investigation, it has been determined that the caregiver was conducting janitorial duties (sweeping/cleaning) while children were in care.

The loose-fill under and around the climbing structure does not measure six inches. Photographs of the equipment and loose-fill were taken at the time of inspection.

The director and did not have transportatin safety training.

The operation did not have a current gas leak inspection.

Documentation showing 7 staff have completed SID's, Shaken Baby and Development training was not available for reveiw at the time of inspection.

Two children did not have current immunization records.
