This standard was found to be deficient. A caregiver was observed to have shaken a child by the shoulders and sat the child down in a harsh manner. This was a self-report, and the caregiver was terminated immediately.

The 2-3 year old classroom teacher was not able to tell how many children she had in care, what their names were, how old they were, etc.
3 children in the 6W-6M classroom were laying and sleeping in cribs with the gate dropped down.
An infant in the 6W-6M classroom was sitting on the bouncy seat while a caregiver was holding the bottle for the child. Child was not capable of holding their own bottle.
The teacher in the 2-3 year old classroom had unsupervised access to children and had no documented orientation training.
Children in the 18-24M classroom had several (3) children with the blankets over their heads.
The 2-3 year old classroom had 1 teacher with 18 children throwing toys at the door, awake during nap time, on top of tables, pulling on exit door, playing over napping children, eating cereal off the sink area, etc.
Three classrooms were observed and over ratio. 10/18/2021- 3 out of 10 classrooms that were observed during the visit were out of ratio. The 0-6 month old class had 2 teachers for 11 infants. The 6-12 month classroom had 7 infants to 1 teacher. The 2-4 year old classroom with a specified age group of 3 years, had 17 children to 1 teacher. 11/2/2021--The 2-3 year old classroom had 18 children with 1 teacher during naptime. Only 3 children were napping, the rest of the children were awake.
A child in the 2-3 year old classroom was walking with a bottle in their mouth. The caregiver stated the bottle did not belong to that child.
The three playground structures at the operation was not observed to have to appropriate depth of loose fill. 10/18/2021- The loose fill was short of 6 inches in the 3 playgrounds (2 yr, 3-6 yr, and 4-6 yr) of the facility. 11/1/2021--The loose fill was short of 6 inches in the 3 playgrounds (2 yr, 3-6 yr, and 4-6 yr) of the facility. Possibly 2-3 inches deep and not 6.
During the visit, a teacher in the 2-4 year old classroom was present while her background check showed pending.
