31 three year old children were being supervised by two caregivers in room 450 during a music activity.

Based on the information gathered, it was determined that one caregiver was supervising 17 three-year-olds in room 450 and one caregiver was supervising 14 children with a specified age group of two years old in room 400.
The operation allowed a caregiver to provide direct care to children in care prior to receiving a background check determination of, Eligible ; Provisional Eligibility with Conditions, ; or, Eligible with Conditions. His/her current status is pending.
Two classrooms contained paper reycle bins filled with garbage. The bins did not contain a lid making garbage accessible to children.
Soiled diapers were accessible to children in suite 400 as the trash can did not contain a lid.
19 children ages 24 to 36 months were fed whole grapes for a snack.
Three caregivers did not know the number of children they were supervising and one caregiver did not know the ages of the children they were supervising.
Children were observed not washing their hands after toileting.
One infant who was awake was left in a restrictive device for more than 15 minutes.
Infants are not being given the opportunity for outdoor activity.
Two infants were sleeping in restrictive devices for an undisclosed period of time.
One employee designated to provide transporatation and who transported children during the summer does not have a current or valid driver license. The deficiency was corrected at inspection. The employee no longer provides transportation at the operation.
Two children were missing DTap, HiB, PCV, and Polio vaccines.
Three infants did not have updated feeding instructions.
This standard was evaluated during the investigation and found to be deficient. The caregiver did not appropriately supervise two children when one child was left unattended in a classroom while the caregiver watched through a window, and the other child was left unattended in a classroom while the caregiver was in another room within the facility.
