This standard was evaluated during the course of the investigation and was found to be deficient. Through interviews with the staff and parent, and through review of sign out logs, it was determined that the parent of a 3 year old child, who was left outside unsupervised, was not notified immediately about the incident. The parent was notified one hour and eight minutes after the incident occurred.
