One caregiver's personnel record lacked the following: notarized affidavit; date of hire; A statement signed and dated by the employee showing he has received a copy of the child-care centers: operational policies and personnel policies; and a statement signed and dated by the employee verifying the date the employee attended training during orientation that includes an overview of your policy on the prevention, recognition, and reporting of child maltreatment. This was corrected at inspection as the operation showed in writing the last date of employment for the employee.
