Through interviews with caregivers, it was determined that a caregiver was not facing an infant and was more than an arm's length away from the infant when the infant fell off a diaper table and received a minor injury.

On the school age playground, the mulch was 1-5 inches in depth in the use zones for the tire swing, ladder to equipment, slide, and two sets of monkey bars. This was corrected at inspection when the operation put up barriers to prevent children from using the equipment.
