Fire drills were not conducted between May to October 2023.

The fire inspection was last conducted 2/7/2023 and is now expired.

Director is missing 13.75 hours of annual training.

The pre-k playground does not have a sufficient depth of 6 inches of mulch under and around the play structure. The schooler playground does not have a sufficient depth of 9 inches of mulch under and around the play structure.

During inspection of the classrooms, I observed four occupied classrooms with dirty, unsanitary areas that were accessible to children. These areas included walls behind the trash where the trash can is, the bathrooms and toilets, and floor area near diaper changing table in the 2's class. The white covering on the particle board beneath the handwashing sink in the school age classroom is breaking and chipping.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

There was a 5 month old infant observed in the crib with 2 toys while sleeping.

An employee did not wash a child hands after diapering.

An employee did not wash her hands after changing a diaper while using gloves.

An employee did not sanitize the diaper changing area after changing a child diaper.

Sink in toddler classroom was being used as a hand washing sink and for bottle/cup washing during inspection. Note: Cups and bottles were moved to kitchen during inspection.
