Based on information gathered through the course of the DFPS investigation, it was determined that appropriate and consistent care was not provided for a child with special needs.

Based on information gathered through external documents and interviews conducted, it was determined that a child with special needs was smacked on the hand by a caregiver while in care.

During the course of the investigation, it was determined that a caregiver has a history of yelling at children in care.

Two children were in the restroom and no caregiver was knowledgeable of the children's were abouts. Multiple children were observed walking in from outside and not received by any caregiver until they were walking out of the door for dismissal.

A child was observed using a ipad during the entire inspection on Youtube. No accommodations forms were available for review.

One caregiver stated, they had approximately 16 children in their group. The caregiver was responsible for 14 children.

It has been determined that the person who is carrying out the responsibilities of a program director of this site is supervising 10 schools.

Children were not prompted to wash their hands after playing on the playground.

Though the drills were conducted, they were not documented appropriately for the months of January or February.

I observed a food allergy emergency plan that was not signed by a physician.

After 7 attempts since May 15, 2022 the operation still has not provided the proof of educational requirements for the site director that has been employed since 2020.
