One caregiver was observed alone iin a classroom with 17 children ages 17 months to 5 years of age. Half of the children were moved to another classroom during the inspection.

One caregiver responsible for 4 infants did not have a complete background check at the time of the inspection. The caregiver was replaced with an eligible caregiver.

A recalled boppy lounger was observed in the infant room being used by a 17-month-old child. This was corrected at inspection when the recalled product was removed from the classroom.
One caregiver stated that they were responsible for 15 children and there were 16 children observed in the classroom. Another caregiver stated that they had 11 children in the classroom and there were 10 children observed.
One infant was observed sleeping in a restrictive device. The caregiver was asked to transfer the infant to a bare crib during the inspection.
During the investigation inspection an individual was present at the operation, with direct access to the children in care, prior to the operation receiving notification from the CBCU that the subject of the background check was eligible to be present.. The individual left while I was present at the operation.
During the investigation inspection it was observed that bottles were being washed in the same sink as hand washing was occurring.
During the investigation inspection it was observed that the building was not being kept in a sanitized manner. Floors were observed to have dirt and crumbs on them. Tables, chairs, and changing tables were observed with visible dirt and not cleaned before use. Toys were also stored on the changing pad in the pre k room. The overall sanitation of the operation was deficient.
During the investigation inspection it was observed that noses were being wiped and the caregiver was not washing their hands after.
During the investigation inspection it was observed that children under the age of two were having screen time.
Caregivers had completed pre-service training in required topics but did not have annual training certificates or hours and caregivers have been here for over 12 months.
During the investigation inspection supervision was observed to be out of compliance. The toddler classroom was left alone while the front door was answered. There were also moments when caregivers would walk out of the room to grab something, leaving the children alone for a few seconds.
Children were brought inside after playing outside and allowed to sit and eat lunch before washing their hands. This was corrected during the inspection when the staff went around the room washing the children's hands off with soap and water.
A caregiver who had children outside playing, brought the children back inside for lunch without washing their hands and proceeded to passing lunch out to the children.
The operations gas leak inspection was due by 12/2021 and has not been conducted.
A person was present at the operation and in care of children who did not have a background check. This person was sent home immediately.
