Observed an infant in a swing and a safety strap was not being used while the infant was swinging. This was corrected when the infant was strapped into the device.

Observed 10 children ages 18-24 months with one caregiver. The classroom was out of ratio by one child. This was corrected when a second teacher went outside.

Observed a classroom that did not have the necessary items to clean the diaper changing station. The operation had been open at 6:00 am and at 10:00 am one classroom still did not have a sanitation solution to clean the changing table. The caregiver was unable to provide information on how the changing table was being cleaned this morning.

While watching video footage of another incident, I observed a caregiver changing an infants diaper and not washing the infants hands. I spoke to the office staff regarding what had been seen.

It was observed in a video that an infant fell, started to cry and the caregiver did not comfort the infant.

A caregiver was placed in an assignment that did not match their skills or abilities.
The refrigerators and freezers had dried food and crumbs on the shelves and where food was stored. The sinks and walls in the bathrooms and classrooms had food on them and were dirty. There was a sink in another room that had a metal curtain rod on the sink. This was removed. In the school age bathroom, there was not a toilet paper holder and the toilet paper was sitting on the trashcan. The toilets were also dirty with yellow stains in the toilets. There were two seats for the children in one room that were stained and had dirt build up on the chairs. There was a broken table in the bathroom of the toddler room and a metal curtain rod on the sink in another bathroom. The table was removed from the bathroom.
The children product safety report had not been completed in the last two years. This was completed during the inspection.
In one classroom there was no toilet paper for the children to have indipendent use. I spoke to the director and caregiver about having toilet paper for the children.
Multiple infant sheets had dirt, stains and looks of dried wet spots on the sheets. The caregivers replaced the sheets with clean ones.
Observed a caregiver grabbing children by the arms, yanking the children, swatting at a child, pulling a child's hair and being rough with the children in care. This was corrected when the caregiver was taken out of the room and removed from the operation.
It was observed on camera that a caregiver placed a child in the bathroom as a form of disicipline. The doorway had a babygate not allowing the child to leave the bathroom.
There was a 14 month old infant in the same room with children who are 3 years old. This was corrected when the child was taken back to the infant room.
There was a dirty diaper sitting on top of the trashcan lid. This was corrected when it was placed in a trashcan with a lid.
Observed a cabinet left unlocked which had chemicals in it. The children in the classroom use the restroom and have access to the chemicals. This was corrected when the chemicals were locked.
A 7 month old infant was sleeping in a bouncy seat. The caregiver removed the infant from the seat as licensing entered the room.
An infant was sitting in a highchair table and was not strapped into the seat. This was corrected when the caregiver removed the child from the seat.
It was found that a caregiver places children in a crib for up to one minute for discipline. Any equipment that a child is not able to freely remove themselves from is considered a restrictive device.
The checklist for the operations outdoor area was not being completed correctly and there were multiple hazards found outside.
Observed the fire drill has not been written down as completed since July 2021. A fire drill was completed but the staff and children were not able to get out of the building in under 3 minutes.
Observed screws sticking out of a bench, the roof of a playhouse not attached to the playhouse but sitting on top of the playhouse.Observed a piece of plastic play climber with a crack down the part of the equipment where the chidren stand.
