Based on the information provided during the course of the investigation, there is sufficient evidence to validate that staff used poor judgment when going in and out of a classroom and not ensuring the door was properly closed. As a result a door was left open and a child left the room.

The First Aid Kit was missing the following: Scissors, , Cotton Balls and Thermometer. This was corrected when the Assistant Director placed the missing items in the First Aid Kit. Corrected during the inspection.
