A 4 month infant had a soft toy book in their arms while sleeping. Inspector removed toy from crib.
During walk through the infant room had 10 infants ranging in age from 4 months to 22 months. Note: A second caregiver did not come in the classroom for approximately 15 minutes.
During my arrival I saw a 5 year old child walked into the center from a transportation van without a caregiver into the center lobby. There was no one in the lobby. There were many times school age children were seen in the hallways without caregivers in the hallways with them.
During inspection, 6 school age children walked into infant classroom with 10 infants present. The youngest infant was 4 months old.
Based on evidence collected during investigation, on March 23rd video footage shows that 2 infants, ages 8 and 18 months were left alone in a classroom for 1 1/2 minutes. The caregiver walked out of the class and then walked back by class 30 seconds later without regard.
Based on evidence collected during the investigation, all parents were not given access to entire center until March 29, 2022.
The director failed to notify parents that an 8 month old and an 18 month old infant were left alone on March 23rd for 1.5 minutes, where the older infant scratched up the 8 month old infant's face. The director did not help caregivers maintain compliance with proper supervision of children. Children were allowed to walk out of the classroom without supervision.
The toddler classroom had 5 bottles/cups not labeled with names of the children it belonged to.
The two slides located on the playground have 4 bolts sticking out of pipes that support the platform of the top of the slide. Each of the bolts stick out approximately 1/2 inch. The bolts are 5 feet off the ground and could catch a child's clothing. A child could also run into these bolts sticking out causing injury.
The mulch surfacing on the playground with the slides is very thin. It appears to be an inch in depth. The side platform on this playground is over 5 1/2 feet tall.
Two caregivers were observed to not have washed their hands after removing their gloves after diapering procedures.

At the time of the incident with children not being unloaded curbside, the driver's transportation training was expired. The date on the training was 03/06/2020.
A bus driver was observed unloading the children in a parking spot and walking across the street instead of the curb that is available at the front of the building.
