While toddler caregiver was taking children to classroom one child was out of site on top of a play house structure in the open gym area approximately 4 feet off the ground. The child could of fallen off the top and caused a serious injury.

Play structure on gym floor with two red slides has two areas with large cracks. One large crack on first platform is approximately 12 inches long in a zig-zag shape with two peaks. Second large crack is deep and circular on second step to one slide.

There is a large hole behind a toilet located in the toddler classroom. There is a pipe going into the wall from the back of the toilet. The hole is approximately 7 inches tall and 6 inches wide. Note: Photo was taken.
There were four napping mats that had tears in them that do not allow them to be waterproof.
One caregiver walked out of the room to take a child to the restroom leaving the other caregiver with 26 children that had a specified age group of 3 years old.
A class was left unsupervised when the caregiver walked into another class to let a child from a different group go to the restroom. Children were also unsupervised when they were allowed to go the restroom in a different room, while the caregiver maintained the rest of the class.

Napping equipment in the naproom did not provide access to the walkway nor did it allow the caregiver to be able to walk inbetween each mat.
The front playground had less than six inches of mulch present and there were weeds that had grown in the playground.

There were three unwrapped water hoses near the playground that pose a tripping hazard.
One caregiver does not have current First Aid nor CPR.
