Staff were interviewed face to face and it was determined no new protocols have been put in place to ensure the safety of the children in care.

After reviewing all of the backgrounds it was found that several persons were no longer with the agency, but there backgrounds were still active.

A report of an adult having a communicable disease/illness was made to licensing 9 days after the required timeframe.

During the course of the investigation it was determined a child was throwing a tantrum and was restrained for this. It was also determined the child was picked up and carried up the stairs and another restraint occurred. No de-escalation methods were attempted and the child was not causing harm to themselves, others, or property.

It was found that foster parent caused a situation to escalate by having an exacerbated tone, arguing, and making inappropriate comments to a child in care to the point where it was reported to be an unsafe situation for the child to remain in the home and the child was placed in respite care and then ultimately discharged.

During the course of this investigation, it was determined that a foster parent slapped/spanked a child in care.

5 out of the 6 children interviewed stated they have been yelled at by the foster parents or have heard the foster parents yelling at other children in care.

During the course of the investigation it was determined a child was spanked by a foster parent causing bruising.

It was determined through interviews that the child in care was getting whooping's by her foster parents.

During the Inspection of the home, it was determined a frequent visitor did not have a valid background check.
