During a DFPS investigation it was found that a caregiver pulled a child by the arm and held them above the ground while carrying the child by the arm and held the child down against their nap mat.

This standard was evaluated during the investigation and found deficient. Two caregivers did not use appropriate judgment when they did not provide an accurate account of how an injury occurred. The standard was in marked into compliance at the time of the inspection. The caregivers involved in the incident were terminated. Retraining on policies and procedures was provided for all staff. All caregivers were observed using appropriate judgment.

Information obtained during the investigation shows that a child was left unsupervised for 2 minutes. This was corrected at inspection. The caregiver was terminated following the incident. All caregivers were observed maintaining visual and auditory awareness, as well as close proximity to the children they were supervising.
