The operation did not follow the guidelines of CBCU, when hiring individuals prior to receiving notice of a clear background check. This was corrected at inspection when the caregiver was removed from the classroom and asked to leave the building. Another caregiver was placed in the room with the children.

From the video footage received, the director and one other caregiver allowed a child to return to the classroom alone and unsupervised, as they continued to another play area. The child can be seen left unattended in a classroom for more than 3 minutes.
