The larger playground had multiple pieces of play equipments with cracks causing shear points. The playground had an airplane structure that had a cracked seat, a nail exposed on the steering wheel, cracks on the propeller, and parts of the structure that were coming apart. The operation stated that children will be kept off this playground until the items can be removed from the playground.
The first aid kit was missing tape and sterile guaze pads.
The operation did not have the required Licensing Parent Notification Poster posted. This was corrected at inspection when the director printed one and posted it during inspection.
Out of 3 staff records reviewed, 2 staff were missing notarized affidavits.
There was not an annual product safety certification available during inspection. The director stated she did not have one and has not certified Form 2885 before.
Based on information obtained during an investigation, the operation did not report to Child Care Regulation within the 48 hours when a child was placed at risk by being unsupervised on two separate occasions. Additionally, the operation did report to Child Care Regulation when a child was placed at risk after receiving an approximate 2-inch linear abrasion below the eye.
A child was put at risk when they were able to leave their classroom on two separate occasions. A classroom was left unattended when the caregiver walked out of the room while the Department of Family and Protective Services (DFPS) investigator was present.
The incident/illness report was not completely filled out. Additionally, the director completed the incident report instead of the caregiver responsible for the child at the time of the incident.
Based on information obtained during an investigation, the operation did not provide an incident report to the parent within the required 48 hours when a child was placed at risk during two separate occasions. On one occasion, an incident report was not created. On another occasion, an incident report was created but not signed by the parent.
Based on information obtained during an investigation, the operation did not notify parents immediately after their child received an approximate 2-inch linear abrasion below the eye. The parents were notified at pick up.

During the follow up there was one crib that had a loose fitted sheet and a child was in the crib. This was corrected at inspection when the caregiver changed the sheet to a fitted sheet.

Out of seven personnel files reviewed two caregivers did not have up to date CPR & First Aid certification.
Medication at the opeartion for a child in care was reviewed and found to be expired.
1 of the 10 children's files reviewed was missing Authorization for Emergency Medical Attention and Transportation.
