As a direct result of the administrator?s failure to ensure timely compliance with the heightened monitoring plan, your operation was unable to successfully move to post-plan monitoring by the original ?planned end date? necessitating previous extension. Furthermore, due to your recent citations issued on March 7, 2025, your operation?s ?planned end date? must now be revised again, and the period of heightened monitoring must be extended again. During an unannounced HM inspection, a home that has a toddler placed was observed to be disorganized and included clutter. This was not isolated in one specific area of the home. The countertops had piles of various items and there was no clear counter space. The dining room table was in disarray with items. In the hallway, there was an office space was in disarray and unorganized with piled items. In the exterior, there were items stored on patio trash can filled with wood, (2) gasoline cans (per foster parent the cans were empty), (5) large metal cans filled with goat food in the backyard. Shed that was open, and door was broken with hinges off. Photographs were taken.
