This standard was evaluated as part of an investigation and found to be deficient. Two school age children were called to the office for dismissal and walked outside to their parents car without being supervised.
Children were observed sitting in bucket seats and a high chair without being strapped in. Any equipment that has a place for straps must be fastened whenever a child is sitting in the equipment.
The last fire inspection was conducted in June, 2020. The director called the fire department during the inspection requesting an inspection and scheduled it for Friday at 10:00.
The health department conducted an audit of immunization records and the operation still has 13 children that they have not received the required immunizations on. The children must not be allowed to come back into care at the operation after the compliance date without current immunization records.

After speaking to the parents, the caregivers and the director at the operation, it appears that supervision was inadequate at the time of the incident.
